My NZREX Journey

My NZREX journey started in February 2016. I studied blindly since there is not enough information about the exam itself. I started by reading John Murtagh's, since some people advised me to do so.

After a month reading John Murtagh's for the second time, (I read this book for my AMC-MCQ preparation in the previous year), I was invited to an NZREX vibre group, which changed my journey into a success story. This vibre group connects some doctors who have passed NZREX and who are still preparing for the exam, to share experience and help each other to pass the exam. There are also regular classes organised through this group to discuss common cases in NZREX. I met some wonderful people in this group and joined a smaller study group which I think is crucial for anyone preparing for NZREX. I started in a study group of three, which later grew to six people and we met two times a week for role-play. I enjoyed practising with this group and learned a lot from more senior doctors in our group and from those who have more experience about NZREX it self. However, after a few weeks I felt that the study pace was not suitable for me, since I was very new to the exam format and need more repetition than the others. Apart from that, I also started an observer-ship in a GP clinic, that made me so busy and I felt tired during the practice time. After a lot of consideration, I decided to take a break from the study group.

In April 2016 I met a surgeon from Fiji that was also sitting her NZREX in November 2016, so we decided to be study buddies. I organised our study plan for the next few months and aimed to finish a month before our exam.

My study plan
  • 1 month of reading John Murtagh
  • 5 weeks history taking practice (aim to finish in 8min/case)
  • 2 weeks counseling practice (aim to finish in 8 min/case)
  • 2 weeks physical exm practice (aim to finish in 8 min/case)
  • 6 weeks of systematic study 
  • Random cases as long as possible
  • Mock exam (4 weeks before the exam)
  • continue with random cases

We met twice a week during history taking practice, and three times a week for physical examination and counceling, and only on Saturday during the systematic study.

I was delighted to receive my result at the end of Novenber and to know that my study buddy also passed the exam. At the moment, I am tutoring another doctor to help her to pass the exam.

Insight to NZREX

NZREX is an OSCE, it is similar to all other OSCE but somehow also very different. It is designed to test wheather you are ready to work in New Zealand as a house officer. Some overseas graduates found it very difficult, as they are not familiar with NZ medical system and culture.

You can practice OSCE like you are preparing for AMC clinical or USMLE step 2 CS, since the cases are quite similar. However, do familiarise your self with the NZREX format and with New Zealand culture since in NZREX the way you communicate with patients is very important. I found that doing an observer-ship is the most effective way to get insight to NZ cultur and to polish-up your communication skills.

The exam consists of 16 stations: four history taking stations, four counceling stations, four physical examination and four static stations. Since 2015 there are some integrated stations included in the exam, which means you need to prepare to perform a focus history taking, focus physical exam and conseling in one station. Time management is key to successfully completing these stations.

Time allocated for each station is 10 minutes with two minutes reading time in between stations. There is a 15 minutes toilet break after the 8th station, and drinking water is provided in static stations without extra time for water break. The only way to pass this exam is to prepare well. The preparation time required will depend on your medical knowledge, your communication skills and how familiar yourself to New Zealand medical system. (read a more complete tips for NZREX here).

Congratulation on passing AMX-MCQ and OET. Unfortunately, if you want to work as a house officer (intern) toward registration as a medical doctor in NZ, you need to pass NZREX and at the moment it is quite difficult to secure a job after NZREX. As NZREX graduates are no longer included in the national matching system.

Hye! I'm an overseas medical student, final year.. I wanted to know what is the eligibilty criteria for NZREX..I have heard that I have to pass ielts then plab1 (or one of few other tests) then I can sit for NZREX.. Is it correct? Plus I wanted to know about specialization scope n process in NZ.. Congratulations for ur success.. Hope u have time to guide me too..

Thanks for reading my blog. The complete requirement can be found in New Zealand Medical Council website.

You do need to pass AMC-MCQ or USMLE 1 & 2 or PLAB, you also need to proof your English ability with academic IELTS or OET.

After NZREX you need to gain general scope registration by completing 2 years as a house officer. Then you apply for specialisation traning. The requirements are vary depend on the specialisation.

Thank you for reading my blog. Unfortunately, it is really hard to predict how long does it take from passing NZREX until you secure the PGY1 job.

It depends on when do you pass your exam, how many NZ medical schools new graduates at that time, where are you applying, and how is your approach to DHBs.

Some people get a job in few months, to a year, but some wait for a few year.

I am so sorry that I don't have a direct answer to your question. There is no guarantee that you can secure a PGY1 position after NZREX. All that you need to do just keep trying.

Good luck for your AMC-MCQ. Keep me posted about your result and your next plan, I would love to hear about it.

It's great to be reading your life journey on your blog. I am glad that you are embarking to becoming registered as a doctor in NZ.

Have you managed to find a vacancy for PGY1 at the DHBs? I understand that it is extremely difficult for NZREX doctors to apply due to the discrimination in their ACE program. It will be great if you can probably share some light in your hunt for a PGY1 position.

Thank you for reading my blog. Unfortunately, I have not found a PGY1 job yet. However I just been looking for about three months and the timing was not right as well as the New Zealand graduates just started they first run in November.

However, lately there are some DHBs that have an open PGY1 position for the next run, so there is a little bit hope here. I will definitely write more about it when I secure a job :).

How about yourself? Are you looking for a job as well or preparing for NZREX?

I decided to do plab 2 instead. I've passed ielts and plab 1 , then came to nz and realized its really difficult to get pgy1 posts after passing nzrex. Hopefully after UK experience it would be easier to get in.

Thanks for reading my blog. I am not sure about your question. Can you verify on what do you want to know?

If you have specialist qualification, you might be eligible for the vocational pathway, please contact the Medical Board of New Zealand directly to know your eligibility. Having said that, most people with specialisation still need to take NZREX.

As for NZREX, there are some requirements that can be found on the MCZN website. here is the link: https://www.mcnz.org.nz/get-registered/registration-exam-nzrex-clinical/#Content-h2-2.

Thank you for reading my blog. At the moment observer-ship is reserved only for those who definitely will sit the NZREX or have passed the exam. So you will need the NXREZ acceptance letter or the NZREX result paper then contact the DHBs directly.

Would you recommend to find a lab job/hospital job (or any kind of job?) in NZ after passing AMC MCQ, to get used to the NZ culture in order to be more familiar with it and try to pass NZREX, or would that be a better idea to do it after passing NZREX while waiting for the match, or simply move to NZ only after getting the match after passing NZREX?

Hi Ava,

Some people worked as researchers or phlebotomist while they were preparing for NZREX, however those are jobs that they already had prior to going down the NZREX pathway. It is very hard to get a job in medical area without any NZ experiences or qualifications. I was not successful to get a clinical job prior to my exam. The other problem is you will need to spare some of your study time to work. So I think if money is not the issue, it is better to focus your time for study. When you start studying you will realise that there are so many things that need to be read and practised.

I highly recommend to move to NZ a few months prior to your exam, and get an observer-ship in NZ hospital. There are some advantages from this. Firstly, you will get familiar with NZ culture and health system, secondly, you can put your observer-ship as a NZ clinical experience in your CV, and get a reference from the consultant. Lastly, you will be able to attend weekly class for NZREX in Auckland, and find study partner to practise. As I wrote in my experience, observer-ship is the best way to get familiar with NZ health system and to boost your communication skills.

Hi Sam,

NZREX graduates are viewed at the same level of New Zealand medical school fresh graduates. That is why we only able to apply for PGY1. However, if you have overseas post graduate experience in the Countries that comparable to New Zealand health system as mention in Medical Council website, then you do not need to sit NZREX and can apply directly as PGY2+.

I'm from Malaysia and just gotten a seat for NZREX clinical next year March. However, I'm starting to prepare now as I'm still on the Nov waitlist.

I am not sure about visa requirement for observership. I suggest you to contact immigration NZ for more information. As I suggest to other overseas candidates, working holiday visa might be the best option if you are eligible. It is worth to contact DHBs first, and when you get the official letter offering for observership you can apply for visa.

I got asked a lot about the job situation. It is a hard question as there are less job for NZREX graduates, so the competition is very high. So it will depend on each individual when they will get a job. There is no guarantee that you will get a job after passing the exam.

As for observership, you can try Waitemata DHB (Waitakere and North Shore Hospital), and, Middlemore Hospital. It is harder to get observership position at the moment, just keep on applying.

Dear Imelda,
This is an excellent blog. I'm a Doctor from sri lanka and planning to migrate to NZ and its very clear that I cant precise as a doctor in NZ immediately. So I'm planning to migrate as a student for a post Graduate diploma in medically relevant field( like public health )in a polytechnic.So i can get working visa for part times as well.During that stay, I'm going to prepare for AMC and NZREX to obtain the registration. I know you have a great experience on job market.
1) what is the best course to study for me to get a well paid job easily ? (it should be a medically relevant field)

2) Is it difficult for IMG who doesn't have a NZ registration, to find a medically relevant job until they pass the exams and get a full registration?

3) I'm also thinking of doing post graduate diplomain health siences, public health or Graduate Diploma in Addiction Alcohol & Drug Studies. Is it a good selection?

Hi doctor,

I think your plan is very good.

I am not familiar with current employment situation in NZ regarding other medical related fields. You might want to contact immigration NZ and the university to gain more information before you decide what to do.

There will be a major change in immigration policy in the near future, so you may want to familiarise your self with it. As for the university studies, not all subjects are offered to the international students. So your option will be limited because of these.

Hi,

It is more and more difficult to get position as first year house officer. If you have completed a specialist training and have experienced in your home country, try to contact the New Zealand Medical Council directly as you may be able to go via specialist pathway.

However most IMGs need to complete NZREX.

Hi Imelda,

One of the requirements for the NZ pathway 2 is that candidates
"have passed NZREX or an international registration examination approved by the Council as being of a comparable standard to NZREX and published on the Council’s website".

what exams are considered comparable standard to NZREX?
is USMLE Step 2 CS or Step 3 considered comparable?
how about Canadian NAC OSCE?

if we have passed any of these exams, would that make us exempt from NAZREX?

Hi David,

the best to do is to visit medical council of New Zealand website and do their self assessment tool.

From my understanding this is what they consider as eligible for comparable pathway (copy and paste from the assessment tool):

1. have unrestricted registration/license for any of the countries listed below, and you have worked as a medical doctor for at least 3 of the last 4 years immediately prior to your application?
- Australia
- Austria
- Belgium
- Canada
- Czech Republic
- Denmark
- Finland
- France
- Germany
- Greece
- Iceland
- Israel
- Italy
- Norway
- Portugal
- Republic of Ireland
- Singapore
- Spain
- Sweden
- Switzerland
- The Netherlands
- United Kingdom
- United States of America

- you must have worked in a comparable system for 30 hours per week, for 33 of the last 48 months
- have full, unrestricted, registration to work in the comparable health system, and
- you have relevant and comparable experience to the position applied for in NZ

First of all, I want to thank you for this amazing website. It is truly helpful for me in this pathway. I have booked the NZREX for March 2018 and I am studying in the Auckland these days.
Secondly, I have two questions:

Regarding the observership, I contacted the DHB directly, for Auckland region Waitemata DHB is the easiest to get observership. However, they might have a long waiting list.

Hi Kyaw,

From what I understand, if you have worked in Australia for at least a year, you can come and work in NZ. However if you don't have any work experience after AMC clinical, you need to sit NZREX.

if someone passed the NZREX, what will be the chance to get a job, and what do you need to do?
I heard that if someone doesn't do observership in NZ, will be impossible to get a hospital position? is that true ?

Hi Sam,

After passing NZREX, one needs to apply for PGY1 position directly to DHBs. As a requirement you need to have three reference. It is easier to get a job after doing observership. But some people get a job without observership.

I'm still looking for a job, unfortunately no luck yet.

If i pass plab 1 and "2" do i still need to sit in NZREX or amc2 to start internship in NZ or AUS?
And Since intern positions are hard to find and its now freaking me out a bit, could you please advise about this matter? Isnt there any area of need maybe or spots with more difficult conditions that made them available?...

Thank you for reading my blog and your kind wishes.
As far as I know you still need to do NZREX after Plab if you want to work in NZ. I am not so sure about Australia, you can check the Australian medical council website for information on this matter.

As for internship position, yes it is getting tougher to secure a job after NZREX, as we can only apply for PGY1 position, and has to compete with other NZ graduates. There are not enough PGY1 job every year for all NZ graduates and NZREX graduates, that's why the competition is so high. There is a recruitment system for NZ graduates, unfortunately NZREX doctors need to apply to the DHBs directly.

Hi Raghav,

Thank you for reading my blog.
Any OSCE book will be helpful to give you ideas on how OSCE is conducted. I particularly like FIRST AID for USMLE step 2cs and Katarina Hurley's book. As for physical exam, I like watching video on youtube on geeky medic channel,add some examination from Talley and O'Connor, then I adapt the examination sequence so it's more suitable for me to finish in 8 min.

You can ask around about the recalls on viber group, to have an idea what the exam focus on, however as the exam is evolving you need to do more research on new managements and guidances on the topics.

I wrote some tips in separate page, you can find the link on my article above. There are some more tips on how to approach the stations on my 'Must Know List' blog posts.

Hi Imelda, Congratulations on your journey. I just recently passed my USMLE step 1 and step 2 ck. I did my observership in the United States. My first plan was there since my brother is already a doctor there. But i had a change of heart since my friend wanted to try New Zealand or Australia and I wanted to try New Zealand. I am from the Philippines and I'm really struggling on how to start. Do i have to be in New Zealand to start my journey? I know people who are trying NZREX already staying there in New Zealand. You don't need to be in NZ to prepare for NZREX, some people prepared overseas and they passed. However finding a study partner to practice for the exam is crucial. If you have no experience of NZ medical system and culture, it can be challenging to pass the exam, as communication is the main thing for NZREX.
However, I have put some blog posts to give you some ideas to start preparing. Feel free to check them out.

Hi Ava,

Personally, I think it doesn't matter how many years after graduation you sit the NZREX, as NZREX is OSCE. I do think the more experience you have working with patient will be more useful for NZREX as it will make you more comfortable doing the exam.

Hi Dean,

The likelihood of getting a job after NZREX is very vary. Some people got job right after exam, some still looking after few years.

If you have specialisation and has work in South Africa for a while, you might be able to do specialisation pathway and needn't to sit AMC or NZREX. Please contact NZ medical council directly for more information and make sure you are on the right pathway.

I guest it's depend on how similar is the way you treat your patient in India compare to New Zealand. Because in NZREX communication is the main thing, so if your approach your patients in India is similar to what they expect from you here in NZ, then yes, 2 years experience will be much benefit. On the other hand, if it so much different, then you may have slight difficulty to adjust and may need more time to practice for the exam.

In NZREX you are expected to show compassion, high degree of respect, clear and effective communication.

Hi Yash,

NZREX is a clinical exam for International medical graduate who want to work in NZ towards registration. If you want to complete a degree in any university in NZ you don't need to sit NZREX.

Prior to sitting NZREX you have to:
- complete your medical degree in your home country at the university that listed in the World Directory of Medical Schools.
- Have passed one of the following within the last 5 years:
- Professional and Linguistic Assessments Board (PLAB) test Part 1
- Australian Medical Council MCQ
- United States Medical Licensing Examination (USMLE) Steps 1 and 2 (Clinical Knowledge).
- Fulfil the English ability requirement.

Hi Kirty,

After passing NZREX you need to apply for PGY1(post graduate year 1) position in New Zealand Hospital. After 2 years working as a house officer you can look up possibility to work as a registrar in any specialisation that you are interested in. Then fulfil all the requirements from any specialisation that you want to be in the training program.

NZREX candidates are not part of ACE program. What are the chances to get JOB outside ACE. I heard it is difficult to get in big cities like Auckland. Is it easy to get first two years in Rural area ?

( any particular DHB which you can recommend ) .

At the moment the chance is very thin, as all the position for PGY1 have been filled by NZREX graduates.

I think applying to all DHBs is the best approach, as you never know which one will have a vacancy in the future.

It's all depend on which DHBs needing additional junior doctors during the year. All hospitals usually full after the 1st quarter intake which start in November. During the next 12 months there may be some empty slot as people for some personal's reasons quit their job or taking maternity or sick leave. This is when we actually have change to get a position in the hospital.

A heartiest congratulation on your success as an IMG. You are leading the right path for us all who are going down the same line. I am Divij, and I have recently cleared my AMC MCQ examination and I'm in 2 minds whether to study for AMC clinical or NZREX. However I am extremely keen to working as a house officer in either of the countries to get familiar with the Medical education system there so that it is easier for me to pass the Clinical exams.

Thank you. Some people said NZREX is easier that AMC clinical. However getting a job in NZ is harder that Australia. You can apply for employment directly to DHBs when you pass the exam.

You might want to check New Zealand Medical Council website for complete requirement.

Basically you will need to pass NZREX and work as house officer to gain registration then apply for training program for any specialisation that you interested in.

I was wondering if having a NZ citizenship would put you in a more advantageous spot when getting the PGY1 spot after NZREX.

Also is NZREX a pass or fail exam? or are there scores?

Hi Jaini,

I believe that having NZ citizenship or PR will give you some advantages. As you do not need to deal with the immigration of NZ to get a working visa.

However there are non resident who manage to secure a job against the residents or citizens. So I can not make further comment regarding to this.

There is no score for NZREX if you pass. You will only be marked as competent. However they use scoring system to pass or fail you on the exam.

I am an IMG and cleared all the USMLE STEPS and have a year experience in United States as an intern/observer anesthesia. I have a specialisation in Anesthesia from my home country. I am preparing for NZREX. However, I will come under skilled visa category but what are the fair chances that I will get employment in NZ? Hi Selena,

I think with your experience in the States will make it slightly easier to get a job here in NZ. However there is no guarantee of getting a job.

I suggest you to ask your supervisors in the States to fill out the reference form that you can download from Auckland Doctors web site.

Get some observer experience in NZ as well, as you need to show that you have knowledge about NZ health system and Maori culture.

I’m a Sri Lankan doctor, currently practicing in Sri Lanka. I’ve done my AMC part one and also my IELTS and am palming to apply for the NZREX available next (March 2019 entry seems to full up so more likely June 2019.)

I’m most likely going to prepare while in SL until a few months before the exam so at the moment I’m looking for study partners and to connect with others studying for the NZREX, both for online study groups and to meet when I get to NZ, most likely Auckland.

Please accept my sincere regards for the great help you are doing to many medical graduates by providing this platform regarding NZREX, an untamed, untraveled and less explored exam indeed.

I am an orthopedic surgeon from India, currently working in the Middle East... since my teenage, I wanted to settle down in NZ... and it hasn’t yet changed... I know it’s a distant dream... but I would like to fancy my chances...

I have cleared PLAB (2019) and USMLE (2007, though did not Go for US residency due to some family issues) - all in 1st attempt

I have booked and been granted to sit in NZREX clinical ... in June 2020...please guide me for the same and if possible, kindly share any link of study groups/online resource for the NZREX.

Hi. I would like to ask and clarify some things I read in the website.
After I sit and successfully pass the NZREX. I would have to look for DHBs/Clinics/Hospitals that would accept me for a position of PGY1? Is that right? Should i have to do it personally or can I apply for the position via email (as I am based in the Philippines as of the moment)?

Also, what part of the process does clinical observership enters? before NZREX or after? Thank you.

It's really inspiring to read your journey and success story. I am an Indian orthopedic surgeon.. appearing for NZREX clinical in June 2021. (previously it was in June 2020, but it's been delayed due to COVID 19 restrictions).

Frankly speaking, I have no clue about NZREX as there are not many resources available. I have an extra experience of OSCE as I have cleared USMLE cs (century ago in 2007 😉)

I would sincerely appreciate if you could kindly provide me a link to some active study group for NZREX...
